Planning Your First Visit

Here’s what you’ll need:

  • Proof of income (1040 tax form, Social Security Benefit Letter, etc.)
  • Your prescribing physician’s contact information
  • A government issued identification card (driver’s license, veteran’s ID, etc.)

Our staff will work with you to ensure you have everything in the necessary format.

Asking for help can be intimidating. MedBank’s staff is patient, understanding and here to help. Upon your arrival, a member of our staff will sit down with you to begin the application process, making sure you have everything necessary to get the medicine you need. MedBank acts as a link between you, your physician and the manufacturer of your prescriptions, taking away the stress of tedious red tape. We print the paperwork for patient assistance programs, then acquire the necessary signatures from your doctor. We then send out applications according to the manufacturer’s specifications. In order to get your medications as quickly as possible, we ask that you come prepared to your first appointment.

MedBank helps eligible patients access large pharmaceutical company Patient Assistance Programs to obtain free medications. Referrals are accepted from physicians, hospitals and social service agencies by fax or mail. MedBank also operates on site services at several area clinics.

MedBank staff and volunteers assist each patient in completing an application for each individual prescription. After applications are signed by the patient and physician, MedBank submits all forms to the appropriate pharmaceutical companies. Each company will send the select medications to the physician’s office, where the patient may pick them up in as little as 10 days. Although MedBank does not provide emergency services, patients released from area hospitals and new patients may qualify for a 30-day supply or until the first shipment of medications arrives for them.
